Searchable list with follow-up questions
Let respondents search a long list, such as medical conditions, and ask separate follow-up questions for every entry they pick.
The Searchable list is a multiple choice field for long lists with hundreds of entries. Respondents type a few letters, pick matching entries and then answer the follow-up questions for each entry separately.
Health questionnaires are the typical case: someone who picks Asthma and Diabetes gets two separate blocks with questions like "Since when?" or "Which medication?".
Add a searchable list
- Open your form under Routing forms.
- Drag the Searchable list field from the Choice group onto your page.
- Under Entries, add one entry per line. You can paste a whole list at once.
- Turn on Allow own entries if respondents may add something that is not on the list.
- Optional: under Exclusive entry, pick an entry such as "None of these". Picking it clears every other entry, and it has no follow-up questions. This lets you make the field required without forcing anyone to name a condition.
When you edit the list later, existing entries keep their follow-up questions. Only deleted lines disappear.
Set up follow-up questions
Follow-up questions live in question blocks. One block can apply to any number of entries.
- Under Follow-up questions, click Add question block.
- Name the block, for example "Respiratory". Only you and your team see the name.
- Add the questions. For each one, pick the answer type (short text, long text, yes / no or date) and whether it is required.
- Under Asked for, search the entries the block applies to, for example "Asthma" and "COPD".
- Optional: turn on Also for all other entries. The block then applies to every entry without its own block and to entries respondents add themselves.
What respondents see
- They type into the search box and pick entries by click or with the arrow keys and Enter.
- Picked entries show as chips and can be removed with the X.
- Below the search, each entry gets its own block of follow-up questions.
- The form only submits once every required follow-up question is answered.
Review the answers
Answers are grouped per entry, for example "Asthma: Since when? 2019 · Which medication? Inhaler". That is how they appear
- in the form's Results,
- in the submission PDF,
- in the CSV and Excel export,
- in the notification email and on the CRM contact.
In the logic of other fields, the searchable list works like a multiple choice: "equals Asthma" is true once Asthma is picked.
Health questions template
The templates include Health questions PKV, a German starter for private health insurance with a list of common conditions, "Keine der genannten" (none of these) as its exclusive entry, and three question blocks. The question is required, so nobody skips it silently. Adjust the list and questions to the insurers you work with.
Health data is specially protected. Ask respondents for explicit consent in the form, for example with a checkbox.
